List of current medications
All inhalers
Spacer or holding chamber (if you have one)
Any recent medical history pertaining to illness (i.e. X-rays, discharge
summary, etc.)
Drivers license and insurance card
If your child is 3 years or older, please try to avoid using any inhalers
the morning of your scheduled appointment. (Please use in cases of
emergency!)
